NorthKey Community Care is seeking a full-time Case Manager for its Intellectual and Development Disabilities (IDD) Crisis Program to monitor crisis support services and facilitate stabilization of crises for clients with intellectual and developmental disabilities (IDD). A bachelor’s degree in a human services field is required, and at least one year of experience working with IDD clients is required.

Qualifications:
  • A bachelor's degree in an acceptable human services or behavioral health field of study from an accredited school is required.
  • At least one year of experience working in community-based intellectual and developmental disabilities (IDD) services.
  • Excellent written and verbal communication skills.
  • A valid driver's license and the ability to operate a motor vehicle.

Job Duties:

  • Collaborate with IDD Crisis Services Team Leader to monitor crisis support services, including crisis responders.
  • Utilize person-centered planning techniques to facilitate prevention and stabilization of crisis situations for clients with intellectual and developmental disabilities.
  • Involve the family, guardian, and providers in planning by reviewing and coordinating the plan of care with them when appropriate and with the client’s consent.
  • Identify and implement appropriate long-term supports and services, including residential placement for IDD individuals as needed.
  • Collaborate with IDD Crisis Services Team Leader to develop a mobile crisis response system for IDD individuals.
  • Assist in identifying, training, and monitoring responders to ensure appropriate emergency response and crisis stabilization.
  • Develop ongoing crisis resources within the local community to meet the immediate and ongoing needs of individuals.
  • Complete all necessary documentation and reports.
  • Collaborate with IDD Crisis Team to provide relevant staff training relating to crisis prevention and response, person-centered planning, and positive behavioral supports within the inter-agency training coalition.
  • Provide outreach to community partners to educate and to enhance crisis prevention and response services within the region.
  • Ensure availability for crisis intervention/emergency referrals, as well as scheduled on-call hours.
  • Maintain HIPAA compliance and appropriate client confidentiality.
