Michelle P Waiver (MPW) Assessment Coordinator- Contracted Position
NorthKey Community Care is seeking a Contract Assessment Coordinator to complete the Michelle P. Waiver Assessment/Reassessments (MAP 351) for participants seeking eligibility for the Michelle P Waiver per NorthKey's contract with DMS. Coordinates process internally, with external agencies, and with families; completes assessments and re-assessments and assures quality of service. The hours will be: Part-Time- Contract Position Between 8:30 AM and 8:30 PM, Monday through Friday. (Hours may be flexible depending on program needs.)

- Bachelor’s Degree in a Human Service field, including psychology, social work, special education, criminal justice, or related field, or a Registered Nurse
- Valid driver’s license and available means of transportation.
JOB DUTIES
- Receives and completes referrals from the MPW Assessment team for participants requiring assessment/re-assessment for the MPW.
- Schedules in-home assessments according to the needs of the family and program requirements
- Completes MAP 351 (assessment form) according to MPW regulations, indicating a detailed description of service and supports the needs of the individual and relevant history.
- Communicates all concerns, questions, or problems regarding the assessment process or regarding the safety, health, and welfare of individuals promptly to the Team Lead of IDD Care Coordination and Performance Monitoring.
- Works cooperatively as a member of MPW case management team to assure quality, efficient services to families and individuals.
- Completes work in a time-efficient manner according to MPW standards and program needs.
- Maintains acceptable productivity standards as established and monitored by the Team Lead of IDD Care Coordination and Performance Monitoring.
- Maintains confidentiality and adheres to HIPPA guidelines regarding each individual’s information.
- Maintains professional conduct and ethical standards.
- Adheres to NorthKey policy and procedures.
