PASRR (Preadmission Screening and Resident Review) Evaluator, Contract
NorthKey Community Care is hiring for a PASRR (Preadmission Screening and Resident Review) Evaluator to complete PASRR Level II evaluations in the community. Contract only (no benefits). Hours will vary and will not exceed 99 hours a month.
PERKS & PAY
- Salary: $28 an hour.
- Will receive a one-time lump sum payment of $250.00 upon completion of all initial training
- Payment request shall be submitted bi-weekly by Provider to the Board by invoice and shall be reviewed and approved as appropriate by the IDD Lead Manager
QUALIFICATIONS
- Bachelor's Degree in a Human Service area, including psychology, social work, special education, criminal justice, or related field.
- Minimum of one (1) year experience working with individuals with Severe Mental Illness (SMI) and/or intellectual and developmental disabilities (IDD).
- Broad knowledge of PASRR case management processes and systems, as well as other community resources, is strongly preferred.
- Valid driver's license, available means of transportation, and ability to drive.
JOB DUTIES
- Schedule evaluations, gather necessary medical, psychological, and personal information about the level of care needs.
- Interpret information and reports in a professional, competent manner to conclude. Assess the appropriate level of care according to PASRR standards and regulations.
- Complete PASRR evaluation reports with a high level of quality and make sound recommendations for care and support services according to individualized needs.
- Complete and submit reports promptly, and communicate with facility staff, SGF Team Leader, PASRR Coordinator, PASRR Services (DDID) staff, and other parties as needed.
- Notify the individual and guardian of the PASRR determination.
- Provide information to the SGF Team Leader and PASRR Coordinator as requested.
- Discuss appropriate alternatives to nursing facility level of care with individuals or guardians, such as Home and Community Based Waiver, Supports for Community Living Waiver, Michelle P. Waiver, etc.
