NorthKey Community Care is seeking a full-time Mental Health Child Case Manager to provide quality and flexible behavioral health case management services to individuals experiencing mental health and substance use symptoms. The position will be based in NorthKey's Covington, Kentucky office. Please note that this position will work with children and their families. You MUST have a bachelor's degree in a human services/social services field.

QUALIFICATIONS

  • Bachelor's degree or above in a behavioral health and/or human services field of study from an accredited school is required.
  • At least one year of experience working with individuals with a history of mental illness and/or substance use.
  • Excellent written and verbal communication skills.
  • Valid driver's license and the ability to operate a motor vehicle.
  • Graduating with a BA is encouraged if you have ONE YEAR of experience in behavioral health.

JOB DUTIES

  • Provide behavioral health care coordination services to children clients and their families who are experiencing symptoms of mental illness and/or substance use as part of a multidisciplinary team of behavioral health professionals.
  • Participate in staffing meetings and consultations with other clinicians to assess the care coordination needs of clients and encourage appropriate referrals to internal and external community services.
  • Assess, plan, implement, and document care coordination services to assigned clients.
  • Act as an advocate to clients to ensure they receive necessary services in the community.
  • Develop knowledge and establish good working relationships with community support resources and services within the community.
  • Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
  • Complete timely and accurate documentation within an electronic health record and other documentation as required by the position.
  • Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills.
  • Maintain flexible hours to meet organizational and client needs.
